Submit an Academic Petition or Appeal

Petition Letter: I am looking to drop this class due to ongoing mental health issues that have prevented my ability complete the courses work to my own standards. I have ongoing mental health issues which led me to drop my classes first semester, in seeking treatment the side effects of different medication have proven extremely detrimental to my mental and physical well-being and having a significant impact on my ability to complete course work. Furthermore, this class specifically has a detrimental effect on my mental health for reasons specific to the course. I did not drop this class before the drop date because I naively felt that I would be able to complete the class, however due to adjustments with my medication in conjunction with not receiving feedback for major assignments after the drop date, I no longer feel that I completing the course would be advisable for my mental or physical health. Please let me know what documentation is needed.
