Petition Letter: Hi there,
I??m requesting academic accommodations for the fall 2022-2023 school year, as I have been placed on academic probation.
My academic performance does not accurately represent what I am capable of nor do I think I was given fair knowledge or awareness of the resources and opportunities there are available to me. Early in the term I experienced brief but psychological traumas, out of my control. In December I was placed on a new medication for my ongoing depression/social anxiety/ and OCD which took a while to become accustomed to. However right as I was wrapping the year there was a passing in the family and although I was able to postpone my exams I struggled with written time limits, and wasn??t able to elaborate on my findings in the time alotted.

Academic Plan: Moving forward I would like to be considered for academic accommodations as I??m currently on a prescription for ADD/ADHD and my written output is hindered due to this medical condition.
I have included a note from my psychotherapist elaborating on my experiences and where we think my academics suffered the most is in timed written responses---in quizzes/exams
I am unable to get psychometric testing at this time due to waitlists, and financial reasons, so I am asking that the document attached will suffice as evidence regarding my cognitive challenges, and I wish to utilize the assets available to me to get my academic success pointed in the right direction.
