Petition Letter: Hello, Associate Dean(s),
This letter is to notify you that up until recently I had been struggling with my mental health leaving me unable to properly complete schoolwork, as well as tend to the responsibilities associated. For the past couple of months since school has ended, I have been building up a strong support system around me as well as strengthening my mindset surrounding academics and my future career. I have been in close contact with my family doctor, as well as gained support from my family and have become comfortable enough to take on the work needed to reconcile for the past school year. In my letter to appeal the request to withdraw you will find an exact depiction of my extenuating circumstances, what has changed to prevent me from slipping again, and my academic plan. I have given and met with my doctor, and she has the doctor's note in her possession, I am just waiting on her to return the form to me. I am deeply sorry that I have not met the deadline to appeal the request to withdraw, however, I strongly believe in reading into my case you will find that I am a smart, driven individual, who has hit a bump in the road but is focused on getting back on track.
