Dear Jenn Stephenson (Associate Dean),
I am submitting an academic petition to request the drop of two (2) failed courses, LLCU 111 Introduction to Cultures and RELS 163 Religion and Popular Culture (Winter, 2021) from my GPA. I apologize for not submitting this petition earlier, and I would like to include a request to waive the 21-day/one-term deadline.
I had extenuating circumstances which began in January 2022, that impacted and temporarily reduced my ability to focus and complete my Winter term course load. At that time, I had a Queens Academic Advisor, and I actively participated with Student Academic Services. As well, I was actively working with a medical practitioner who assisted me through my ordeal.
I have attached a support letter from my medical practitioner, Pam Wilansky.

Academic Plan
Academic Plan: For the coming 2022-2023 year, I have prepared an academic plan which includes a focus on Sociology courses because it is a passion and area of interest of mine. I am registered in 4 courses (instead of 5) for each term and I made good use of and will continue to do so with TAs, I have also hired a personal tutor to assist me with chunking the components of my assignments as per my learning profile.
I am already registered with Student Success Services (Fall 2021) for accommodations, and I have a solid support network to continue my studies and achieve success at Queens. These supports are in place, and I am committed to improving my academic performance.
As a matter of interest, this Summer, I enjoyed full-time employment as a Hostess and a Camp Counsellor, and I am ready and prepared to return to Queens on a full-time basis.
