Petition Letter: To Whom It May Concern,
This past November, I received a blow to the head and was given a concussion, the effects of which I still suffer to this day. Due to this, I have struggled with emotional and physical pain, as well as cognitive difficulties all throughout my second year.
Back when the injury initially happened, I had received an extension for one of the tests for this class, MUSC 129, and I had agreed to complete it a few weeks ago. However, due to illness (concussion related headache and exhaustion, which my prescribed medication couldn¿¿t control), I was unable to attend the re-test, and my professor submitted the test as a 0, causing me to fail the class.
When I saw my failed grade on Solus, I promptly emailed both my professor for the class, Dr. Tormann, and academic advisor, Dr. Lind. They both encouraged me to submit an appeal in order to change my grade. My hope with this appeal is that my grade can be shifted to increase the weight/ percentage of the successfully completed tests, and allow me to pass the course. This incomplete test is an equal weight to the other tests required in this course.
This entire year has become a series of unfortunate events due to my injury, and my academics have already suffered greatly. I truly wish to avoid failing yet another course, and having to take the full year course once again, due to this one incomplete test.
In this appeal, I am submitting my most recent doctor's note from my concussion specialist, Dr. Michael O' Connor. Though the note references an extension of an assignment for a different course, it is the most recent documentation regarding the treatment of my injury. If necessary, I can obtain another doctor's note for the purpose of this appeal. In addition, both Dr. O' Connor and my advisor through Queen's Disability Services, Mr. Michael Beganyi, can be contacted if any further investigation is necessary.
I truly appreciate you taking the time to read my appeal, and I hope that this may be resolved, and that my grade shifted to allow me to pass the course.
