Decision Letter

Appeal Number: 1867
Decision Date: April 19, 2016
Associate Dean: Sue Blake
Student Name: Rachel Song
Student Email: 8sys@queensu.ca

Dear Ms. Song:

I have carefully considered your request to have your appeal heard after the 21-day timeline for submission, and have decided, based on the circumstances you refer to in your letter of appeal, to waive the timeline. You may now proceed with your planned appeal to waive the requirement to withdraw for a minimum of three years.

Please keep in mind that for this appeal to be successful you must provide the timeline of the extenuating circumstances that prevented you from completing your 2015 winter term courses, along with an explanation of how these circumstances affected your academic progress. Documentation supporting this timeline and severity of impact must also be provided.

It will also be necessary to provide evidence that your circumstances are being actively managed so that we have strong reason to believe that a return to studies at this time will be successful.

Please access on the on-line appeals system at: http://www.queensu.ca/artsci/help/academic-appeals/online-appeal-submission After you enter your NetID and password you will now have a new appeal option called €œSubmit a Late Appeal€. Choose this option and follow the steps. Your appeal letter should explain the extenuating circumstances concerning these particular courses, and include any relevant supporting documentation that wasn€™t included with your prior appeal. There will not be a fee for this new appeal, as it will be considered an amendment to your 21-day deadline appeal so the system will skip over the payment step.

If you have any questions, please feel free to contact the Faculty Office for advice or assistance.

Sincerely,

Sue Blake
