Decision Letter

Appeal Number: 2577
Decision Date: September 12, 2016
Associate Dean: Sue Blake
Student Name: Matthew Azan
Student Email: 14ma42@queensu.ca

Dear Mr. Azan:

I have carefully considered your request to have your appeal heard after the 21-day timeline for submission. Based on the circumstances you refer to in your letter of appeal, I have decided to waive the timeline for the 2015/16 year and grant your appeal to drop PHYS 242 (F 2015), BIOL 341, CHEM 282, MATH 124B (W 2016) and CHEM 282 (S 2016) past the academic deadline.

I will however, deny your request to receive Credit standing in BIOL 303 (W 2016). There is no evidence supplied in your appeal or in your file to confirm that your grade in this course would have been higher, had you received academic accommodations, given that there are many grades in the D to C+ range on your transcript.

In lieu of dropping other failed courses and receiving Credit standing from earlier years, I will instead waive the Requirement to Withdraw for One Year so you can study for the next year with the recommended accommodations put into place. This will allow you to raise your GPA to 1.60 and return to good academic standing. With the above courses dropped, your current GPA will remain at 1.56.

Please read carefully the document to follow this letter. It contains important information concerning your current academic standing. If you have any questions about the information, please contact the Faculty Office to schedule an appointment with an academic advisor.

Upon returning to a regular course of study it is your responsibility to choose courses carefully, seek out the advice necessary to improve your academic performance, and maintain a GPA consistent with the requirements for successful completion of your plan. I hope that you will take advantage of the variety of support services available to you at Queen's. Academic counselling is available through an advisor in the Faculty of Arts and Science Student Services Office (main floor Dunning Hall, phone 533-2470) or through your plan department. The Learning Commons in Stauffer Library also provides a range of services, including workshops designed to improve study skills and develop strategies for meeting the demands of academic life. Further, the Student Wellness Services Centre located in the LaSalle Building at 146 Stuart Street (613-533-2506) offers personal and academic counselling free of charge to all Queen's students, as well as individual consultation to aid in coping with difficulties of a more personal nature.

Note that any change in academic registration may impact government or private-firm student financial assistance eligibility and/or funding. Please consult with the Student Awards Office, Office of the University Registrar, Gordon Hall, to understand the potential implications of changes to course or program registration and/or the consequences of not successfully completing the required course load.

If you wish to appeal the Associate Dean€™s decision to the Board of Studies, you must submit an appeal to the Board of Studies within 21 days of the date this decision was sent.
