Dear Ms. Hinch:
I have carefully considered your appeal to drop PSYC 205, PSYC 241 and PHGY 350 (F 2013) past the academic deadline as well as waive the requirement to withdraw for a minimum of three years. After reviewing your file and the supporting documentation you provided, I have decided to grant your request with a special condition.
Due to the nature of your difficulties and your current academic situation, I have made the decision to place you €œPermission of the Associate Dean (Studies) Required to Register€. Prior to registering in courses each year, you will be required to set up an appointment time to meet with me so we can review the progress you€™ve made over the previous terms and your academic plan for the upcoming terms. I will also ask that you meet with an academic advisor prior to the fall and winter drop deadlines to ensure that you are managing your course load well and reviewing your academic options. It is my hope that closely monitoring your academic progress over the next year will help you to remedy the personal and academic difficulties you have faced in the past and thereby enable you to be more successful in your future studies.
If you wish to appeal this decision to the Board of Studies, you must submit a letter of intent to appeal within 21 days of receiving this decision. Appeal forms are available from this office (F200 Mackintosh-
Corry Hall) or on the web at http://www.queensu.ca/artsci/sites/default/files/appboardstudies.pdf. General information on appeals is available at http://www.queensu.ca/artsci/academics/academic-appeals/students. You may contact Mr. Harry Smith, Dispute Resolution Mechanisms Coordinator for advice on matters of procedure and on obtaining a representative. His office is located in room 153 Richardson Hall and he can be reached by phone (533-6495) or e-mail (drm@queensu.ca).
In returning to a regular course of study it is your responsibility to choose courses carefully, seek out the advice necessary to improve your academic performance, and maintain an average consistent with the requirements for successful completion of your program. I hope that you will take advantage of the variety of support services available to you at Queen's. Academic counselling is available through an advisor in
the Faculty of Arts and Science Student Services Office (F200 Mackintosh-Corry Hall, phone 533-2470) or through your department of concentration. The Learning Commons in Stauffer Library also provides a range of services, including workshops designed to improve study skills and develop strategies for meeting
the demands of academic life. Further, the Health, Counselling and Disabilities Centre located in the LaSalle Building at 146 Stuart Street offers personal and academic counselling free of charge to all Queen's students, as well as individual consultation to aid in coping with difficulties of a more personal nature.
If you have any questions about this decision, please feel free to contact the Faculty Office for advice or assistance. I wish you success in your future studies.
Yours sincerely,
J. Hugh Horton, Ph.D.
