I am sorry to read in your appeal of the passing of your grandmother as well as the health challenges you have had this term. I also noted that you made attempts to seek help from your doctor at Student Wellness prior to the drop date but was unable to arrange an appointment. In light of the extenuating circumstances you have outlined, as well as your timely appeal, I have decided to grant your request to drop PHIL 271 (W 2019) past the academic deadline.

I am pleased to see that you have created a strategy to ensure you meet your academic obligations going forward, and that registering with Queen€™s Student Accessibility Services (QSAS) is part of that strategy. With their assistance, you should receive the appropriate accommodations to help lead you to successful outcomes in your courses.
