I acknowledge that you have been connected with professional health-care supports since the 2022 Fall term, and I have granted this petition due to the academic impacts of your extenuating circumstances as confirmed by the documentation that you supplied. Your grades in these courses will be replaced with a DR notation on your transcript.

Upon your return to studies, if you find that your extenuating circumstances continue to impact your courses, it is your responsibility to evaluate your academic progress before the drop deadline of the term and make sure to drop the course if you have fallen behind or find the material too difficult in it.
