Dear Patrick,
I have reviewed your request to waive the 1-year petition deadline related to the 2022 Winter term. After reviewing your file, I have decided to grant your request.
I acknowledge that you were not able to submit this petition within the 1-year petition deadline and I suggest that you consider registering with Queen’s Student Accessibility Services (QSAS) if you have not done so already, so that you obtain ongoing support from a Counsellor who can help manage the ongoing impacts of your health on your academics.
As the documentation that you supplied confirms an unaccommodated disability that you were not able to seek support for until after the 2022 term ended, I will allow you to drop PHYS 317, PHYS 345 and PHYS 372 (W22). Your grades in these courses will be replaced with a DR notation on your transcript.
I will also allow you Aegrotat Standing in PHYS 350 (FW21/22) as I acknowledge that you were not able to complete this course through an Incomplete Grade during the 2022 Summer term due to the ongoing impacts of your extenuating circumstances. Your instructor has supplied an Aegrotat Estimated Grade of A which will appear on your transcript.
Note that any change in academic registration may impact government or private-firm student financial assistance eligibility and/or funding. Please consult with the Student Awards Office, Office of the University Registrar, Gordon Hall, to understand the potential implications of changes to course or program registration and/or the consequences of not successfully completing the required course load.
Sincerely,
Jenn Stephenson, Ph.D.
