Documentation can include, but is not limited to, a letter from a professional (such as Student Wellness Services and/or Queen’s Student Accessibility Services, an Off-Campus Physician’s Letter https://www.queensu.ca/artsci/sites/faswww/files/form/off-campus-physician-note.pdf (if your healthcare provider is not within the Queen’s community), death certificates, legal documents, etc. If your extenuating circumstances are personal in nature or cannot be documented professionally, we may accept a letter from a family member or friend if they supply their contact information so that it can be verified.
