It would also be helpful for you to comment on the current state of your extenuating circumstances if they have since resolved or are being managed in a way that would allow you to be successful in future classes.
Your petition must also include an Academic Plan. Things to consider when preparing a plan for your return to studies are reducing your course load, repeating courses, utilizing academic resources on campus, meeting with a personal counsellor, etc.
Your documentation should confirm the timeline and severity of the extenuating circumstances (if it is possible to determine) and how the extenuating circumstances impacted your academic ability in your courses. Documentation can include, but is not limited to, a letter from a professional (such as Student Wellness Services and/or Queen’s Student Accessibility Services, an Off-Campus Physician’s Letter https://www.queensu.ca/artsci/sites/faswww/files/form/off-campus-physician-note.pdf (if your healthcare provider is not within the Queen’s community), death certificates, legal documents, etc. If your extenuating circumstances are personal in nature or cannot be documented professionally, we may accept a letter from a family member or friend if they supply their contact information so that it can be verified.
