Petition Letter
Due to extenuating circumstances, beyond my control, I was unable to continue working on my abovementioned courses beyond March 9, 2023. From that date onward, my mental health deteriorated abruptly and significantly to the point that I could not think clearly sufficiently to continue my studies or complete many seemingly simple daily tasks. On March 9, 2023, (with the assistance from my parents) I reached out to my psychiatrist and psychologist. Although my medication has been altered and my therapy sessions have increased in frequency, at this point in time, I??m still unable to study. I have been told by my psychiatrist that it will likely take over a month before my condition improves and I am able to return to my studies. It is not entirely certain, at this point, when I will be sufficiently better to continue my studies, but I am determined to keep working with my health-care professionals on a regular basis so that I can return to my studies.

1. Off Campus Physician Note completed by psychiatrist Dr Zemans, and
2. Letter from Dr Zemans confirming my mental health deteriorated as indicated above and confirming ongoing regular appointments
