Petition Letter
To the Associate Dean (Academic) of the Faculty of Arts and Science,
I am writing to request to drop the course PHYS460 (Winter, 2023), and to request an extension for an IN grade for CISC124 (Fall, 2022) into the Spring/Summer 2023 term.
I have been experiencing an exacerbation of an ongoing health condition. During the Fall 2022 term, this exacerbation mostly affected my physical health (and some mental health) which severely limited my capacity. I have since mostly resolved the physical health problems, however the mental health condition increased in severity (inability to focus, elevated levels of anxiety affecting executive functioning, and more). This exacerbation has prevented me from completing all coursework in the Winter 2023 term (PHYS460 and CISC124). I have been working with a mental health counsellor and QSAS advisor since I recognized the severity of the condition late in the term.
I am hoping to complete coursework for CISC124 this July/August after a couple months away from school to work on my health with family support at home. My CISC124 professor has created an alternative plan with a reduced load to help me complete the course requirements during the summer term; he is in support of the IN extension. I have also connected with a learning strategist through SASS and my family doctor for further support.
I have included documentation from a mental health practitioner with whom I’ve been working with who can speak to the circumstances outlined above. Please let me know if you require further details.
