Petition Letter
To Office of the Associate Dean,
My name is Siyi Emily Bao and I am a student at the Faculty of Computing.
I am writing this appeal as a request that my MUSC 102 course not to be counted for my cumulative GPA.
I have been suffering severe dysmenorrhea ever since my first menstrual cycle. It occurs every 39-50 days which is very irregular. One or two days before and during my period I experience severe headache, vomiting and dizziness, fatigue feeling associated with low fever which don't allow me to concentrate and study. My MUSC 102 exam happened to be in this period. During the exam, I had to ask the teacher who monitored the exam to leave for the washroom.
The previous occurrence of dysmenorrhea I experienced was in the library on March 10th. I had to lie down on the library floor and later called the First Aid from the campus for help. If you check the phone record, you will find that I called via my cellphone (613-770-7096) around 2pm on March 10.
My irregular period and my health condition had a negative impact on my preparation for and during this exam.
Please find attached the medical certificate from my family physician.
