To Whom It May Concern,
I am applying for Late Withdrawal accommodations from the Fall 2023 semester. I have had several medical and mental health issues this semester that resulted in me being hospitalized at the end of November. These past couple of weeks I have been recuperating and going to many different doctors appointments across several different cities. i have been diagnosed with several new chronic conditions, the worst being the constant migraines we did not know were affecting me in such debilitating ways. I expect I will need to take another semester to recover before returning. Please find attached a letter from my physician from the hospital, who supports my application to withdraw from school for medical reasons.
