To whom it may concern,
I am appealing to respectfully request to drop all courses retroactively from my 2014-2015 academic year.
I have suffered from depression for the past six years and my struggles with mental illness spiralled dramatically this past year which tremendously affected my academic performance to an extreme degree. As my psychological challenges continued to increase, my failure to speak out about my issues, irregular sleeping patterns and poor diet affected my mental health and my academic performance continued to suffer. In addition, I have a history of multiple concussions and an accident in February 2015 caused me to suffer from my eighth concussion as well as seizures, which added to my stress and debilitating depression limiting any improvement in my mental health. As a result of my silence and mental health issues I politely request to have all of my courses dropped and my failing marks expunged from my records.
I plan to spend this coming academic year at home with my family in a stable and supportive environment while continuing medical treatment which began as the result of a hospital admission in May.
My plan for the future includes strengthening my mental health while strongly re-evaluating my life goals and continuing to pursue my education.
