To whom it may concern,
I am writing to formally request an extension for completing the requirements for CHEM 311 due to extenuating medical circumstances that significantly impacted my ability to prepare for and complete the deferred final exam within the allotted time frame.
On Friday, December 8, 2023, the day before my scheduled final exam for CHEM 311, I began feeling ill and experienced severe exacerbated symptoms that rendered me unable to attend the examination. It is important to note that I have been diagnosed with anxiety, ADHD, and depression, and I show signs of panic disorder. These conditions, coupled with the physical illnesses I encountered, significantly magnified the challenges I faced during this period. The combination of severe physical illness and my ongoing mental health challenges led to an overwhelming situation that impaired my ability to study and concentrate, further hindering my preparation for the deferred CHEM 311 final exam.
A visit to the walk-in clinic on December 15 confirmed that I had contracted strep throat, and I was immediately prescribed antibiotics. Unfortunately, after seven days into the necessary ten-day treatment, I developed a severe allergic reaction to the medication, resulting in a full-body rash and hives that lasted approximately 2-3 weeks. This reaction led to exacerbated symptoms related to my mental health and compromised my immune system, leading to prolonged periods of exhaustion where I found myself sleeping for 15-20 hours a day.
This series of health issues was further complicated by the onset of my menstrual cycle, which exacerbated my already weakened immune state, leading to a secondary illness. The cumulative effect of these health challenges severely affected my ability to study, concentrate, and prepare for the deferred CHEM 311 final exam. This also led to exacerbated symptoms related to my mental health.
Attached to this letter, you will find supporting documentation, including medical certificates from the walk-in clinics that attended to me, detailing the timeline and severity of my conditions. These documents attest to the genuine and serious nature of my health issues during this period.
Given these circumstances, I am requesting an extension for my incomplete grade in CHEM 311. This additional time will allow me to recover fully and prepare adequately for the deferred exam, ensuring that my academic performance is not unduly affected by these unforeseen and uncontrollable health issues.
I understand the policies regarding incomplete grades and the importance of adhering to academic deadlines. However, I believe that my situation constitutes a significant and valid exception. I am committed to completing the requirements for CHEM 311 as soon as I am able to do so. Throughout this challenging period, I have remained in contact with the professor responsible for CHEM 311, seeking to establish a definitive date for the deferred final exam. However, as of this writing, I have not received a confirmed date.
