Petition Letter
Dear Sir or Madam:
I would like to apply for an Aegrotat Standing appeal in the course HLTH 334. I have been suffering from depression and anxiety intermittently over the last four years and this past winter term my illnesses worsened significantly.
With the support of my family doctor, Dr.Lorie White, my previous psychiatrist Dr.Lynda Molleken, and my current psychiatrist Dr.Brian Macdonald, I was able to defer my final exams. By mid-June, Dr.Macdonald and I agreed that my treatment was going well enough for me to attempt to write my final exams.
During my HLTH 334 exam I suffered a severe anxiety attack and was unable to function for 1.5 hours, about half of the given time. As a result I have received a C+ on the exam. In addition to the final exam I have been attempting to complete an outstanding assignment for the last five months.
Due to ongoing impaired concentration and academic functioning, I have yet to do so successfully.
The SKHS Undergraduate Coordinator, Anna van der Meulen, has advised me to appeal for Aegrotat Standing because I have completed over 60 of the course work and both my performance on the final exam and in writing my final essay have been negatively impacted by my illnesses. If granted this appeal, I will receive a B- as my final mark. I believe this is the best course of action for me because I am confident that if I attempt to complete the course, by submitting the assignment, my final grade with decline.
I will attach documentation from each of the listed doctors confirming my illness. The notes were written over the course of the spring and early summer.
The form only allows me to submit three notes, however I do have a fourth written by Dr.White. If you require it for your decision-making process please let me know and I will send it separately.
