Dear Sir/Madam:
I am writing to request a withdrawal (drop) from my fall semester courses (GEOL 104 and GPHY 104). I have been experiencing significant health issues that have resulted in admission to hospital here in Toronto where I am still a patient. I have not been well enough to manage my personal affairs which is why this request is only just being submitted.
I am attaching a letter from my doctor and can provide further support if necessary. Once my treatment plan is in place, I have every hope and confidence that I can return to my studies in January. I have registered online for my courses. I will reestablish my accommodations with the DSO and I will clarify who will be my academic advisor for next term. I promise to seek out and accept the necessary supports for my success.
I am now home and my doctor supports my return in January, with support. I would truly appreciate an opportunity to continue my studies at Queen¿¿s in 2016.
