Dear Madame/Sir,
I am currently appealing to drop my courses for the winter semester after the academic deadline. I am asking the Associate Dean to accept my request since the condition affecting me is medical in nature as substantiated by my family doctor and subject matter medical experts. I note the devastating nature of this disease which prevented me from attending classes and forced me to retreat to Oakville to obtain medical help. ¿¿ Specifically I have been unable to attend classes or perform any academic work since the end of February 2016; I have been in Oakville, away from Kingston and Queens obtaining medical help and treatment for this condition.
I have been medically treated for anxiety and depression which increased in severity over the last 2 years. This medical condition caused me difficulties in first year courses as well as in courses for the first semester of second year. I hoped the medical treatment received to that point would enable me to continue, however since the proper diagnosis and treatment of mental health issues can sometimes be a matter of trial and error it has taken my doctors up until this point to determine that I should instead be treated for bipolar disorder. This requires a different type of treatment and medication entirely, and is the reason why I have not been able to fulfill my academic potential.
To ensure this does not occur again I am being more closely monitored medically to confirm the latest diagnosis and treatment and accurate and appropriate in my specific instance.
It is my hope with this new treatment I will be able to start studying again, however, I ask that my winter 2016 falls be removed from my academic record and that I be reimbursed the course fees. I have attached medical documentation from my doctors, please do not hesitate to contact either them or me if you have any remaining questions or concerns.
