Dear Sir / Madam;
REQUEST:
I am writing to request the following: I seek aegrotat standing in HIST 122 and PSYC 100, and a late drop of CLST 103.
EXTENUATING CIRCUMSTANCES:
On March 17, 2017 I suffered a concussion (my third in recent years). I immediately felt ill, having severe and maintained headaches. I was essentially bedridden for not less than a week. Thereafter I attended to Kingston General Hospital and was referred to Dr. Michael O'Connor at Kingston Orthopeadic Pain Institute. I enclose a copy of his correspondence.
This injury severely impaired my ability to not only concentrate on my courses, but attend courses and comprehend reading materials. I have been continuing to take heed of Dr. O'Connor's recommendations, and do attempt to read and perform academically, however my ability at this time is quite restricted.
As a result, my ability to effectively, and to the best of my ability, write and attend the final examinations in HIST 122 AND PSYC 100 are effectively compromised to the extent that I seek aegrotat standing in those courses.
Prior to my injury, I have not been performing particularly well in CLST 103. I had spoken to my parents and peers and had decided that I would turn a significant amount of my attention to that course and increase my grade therein. However, the injury occurred prior to the third term exam, and consequently I was and am unable to perform academically well in that course. Therefore, I seek the late dropping of that course.
FUTURE PLAN:
As this is my first year at Queens, I am quite distraught over the injury I have suffered and its academic impact it has had upon me. I have seen an academic counselor and I am confident that with restricted, cautious actions over the next 6 to 8 weeks, including continuing to perform academically as best I can, I will fully recover and be ready for the fall 2017 term.
I have sought out aid from the Queen's Student Accessibility Services, and with their help I have set in place a plan for my current courses. As well, upon returning to school in the fall 2017 term, I will further seek out their help in ensuring that I can perform to the best of my abilities and succeed in my planned full course load.
I also going to keep under that care of Dr. O'Connor and maintain an ongoing relationship with my academic counselor in the near term, to support my future academic pursuits. My grades thus far at Queens have been between 75 and 85. I feel that without suffering this injury, those grades would remain at least consistent.
