Submit an Academic Petition or Appeal

Due to traumatic medical circumstances, my concentration, well being and focus has been greatly hindered over the past semester. Due to this I have been unable to perform as well as usual and thus it has had a significant impact on my grades. I am more than willing to send additional medical information and/or documentation if required. I have been seeing counsellors and other medical professionals weekly, often times more than once a week to try and fully recover as swift as possible.
