To Whom It May Concern:
My name is Nicolai Bopp; I am an undergraduate student currently serving my requirement to withdraw for one academic year. My reason for submitting this form to waive the 21-day deadline is during the allotted 21-day period to submit, I did not have the appropriate medical documentation to support my struggles with ADHD as well as a possible learning disability. After working with my health care provider for a few months, I have started a prescription as of July 22nd and am currently in the process of completing a psycho educational assessment that is planned to be completed by the first week of October. Please find attached a letter from my psychologist, as well as documentation of prescribed medication.
