I am an undergraduate student in School of Kinesiology and Health Studies (Phys. Ed.). Last spring, at the end of my second year in the program, I experienced major health issues that affected my ability to focus on my schoolwork. Unfortunately, I am still sick and cannot return to my studies at the moment.
I am requesting the following:
1.Revise my course mark in ANAT316 (Human Visceral Systems) to show as CREDIT on my transcript.
2.Receive an AEGROTAT standing in KNPE 227 (Exercise Physiology) and KNPE 254 (Biomechanical Analysis of Human Movement).
I took these courses during the winter term, with exams occurring in April, 2018.
Throughout the Winter 2018 term I felt progressively more unwell. By the end of term, during exam time, my health had deteriorated to the point where I could not complete all my exams. The last exam I wrote was ANAT 316, but I struggled to even get to the exam, let alone express what I knew of the material.
I initially sought help from the Wellness Centre at Queen¿¿s in April, but then returned home to be attended by my family doctor. My illness and consequent return home caused me to miss my final two exams, and I initially asked for accommodation to postpose those exams to September 2018. I had completed course work for these classes, prior to missing the exams. Though I have seen a series of specialists since then, more than four months later, my symptoms persist to the point where I cannot function normally. My illness makes it hard for me to concentrate. I tire very easily as well as feeling physically very ill. I am still undergoing more tests to determine the best course of treatment as initial treatments have been unsuccessful.
My goal is to return to my studies at Queen¿¿s, but I am taking a break at least for the Fall 2018 term, and perhaps longer, to focus on my health. As my health improves I plan to return, likely with a lighter course load to start, building up to a full course load once my health allows. Just before I got sick, I was considering switching programs within Arts & Science. I have decided to put that decision off until I am well, as I am not in a frame of mind to be able to make it now. In talking with my academic advisor, I understand that the required PACT333 will be discontinued in 19/20, and that if I am unable to complete it, I will need to ask to substitute another course to fulfill my plan requirements. I will contact an academic advisor to work out a plan when I am well enough to return.
