Petition Letter
To Whom It May Concern,
I have found myself struggling with the symptoms of severe depression, including cognitive challenges, and am unable to focus on or complete the required assignments and readings. I thought I was better, but have found that my symptoms have worsened again recently, and it has resulted in being unable to formulate thoughts and write coherently, as well as causing severe anxiety. Additionally, I am caring for my husband who is recovering from cancer and chemotherapy, as well as a life-threatening bleed in June that has left him unable to walk properly.
I would love to take the course when I am better, but know that I cannot at this time.
I have been working with my psychiatrist, who is aware of my relapse and my request to drop this course. I have a note from her for work not completed a couple of weeks ago, and can obtain another one if/when needed.
Thank you for your time,
Sincerely,
Rochelle Collins
