Petition Letter
Dear Dean of Associate Studies,
I am appealing to waive the 21-day deadline for the appeal for courses taken during the semester of Winter 2016, 2017, and 2018. For majority of my academic career, I have dealt with mental health obstacles that have made meeting academic requirements and writing exams difficult. In 2016, personal life events, financial and academic stressors triggered severe depressive episode leading to self-harming tendencies and isolation. It was not until seeking help in the summer of 2017, meeting several doctors and counselors, and getting referred to an outpatient program at Hotel Dieu was I, in Fall of 2019, suggested to appeal for my grades by the QSAS (Queen's Student Accessibility Services). I have attached the documentations, provided by the doctors and mental health team at Hotel Dieu and counselors from the university. Thank you for taking the time to read this appeal, I highly appreciate it.
Sincerely,
Sarah Slim
