Petition Letter
To whom this may concern,
My name is Cam Tu Le. I am currently working towards my Bachelor's of Life Science at Queen's. I am writing to you today to request my spring/summer course PSYC 100 be dropped from my record without academic penalty. I have been suffering from depression since the beginning of the year which caused me to drop my courses for the winter 2019 semester. Believing that I could finish PSYC 100 during the Spring/Summer Semester, I enrolled in the course and was doing very well in grades until mid-term hit when my mental health declined rapidly. I took a leave of absence from work at the end of June to relieve some of the stress to finish the course because I didn't want to repeat the course for the third time. Unfortunately, the LOA didn't work and after a suicide attempt at the beginning of July, I realized I needed help and decided to seek help from my doctor. Attached is a document provided by my doctor that outlines my visits to the doctor's office, emergency room, psychiatrist and treatment. During July, I wasn't in the state of mind to study or work through the course load because the symptoms that I was experiencing left me unable to perform normal daily activities. I hope that I provided enough information to have my request to be approved.
