Hello, My Name Is Tyler Gay
I am looking to get 4 courses removed from my transcript due to my extenuating circumstance. The courses In question are ECON 212, ECON 222, ANAT 100, and BIOL 102.
I am a student with severe ADHD and dyslexia, because of this i do have accommodation to help me with my courses however at the start of 2019 when i returned to school, the medical staff here at Queens refused to supply me with my required medication (previously taken for years). They would only provide a new dosage and new version. What you need to understand is when it comes to medication for ADHD and dyslexia is that they are no direct substitutes and everyone requirements are different. I thought I would be able to push though it but I clearly was not able to do so and therefore failed several courses. Because I was born and raised in Mexico City it was not easy to get my medication from my family Doctors. It was not until mid October that I was able to fly down and meet with him in order to received my required medication. I was then made aware of this appeal process for which i reacquired proof. I was recommended by the Arts SCI office to obtain proof of this circumstance before submitting a appeal so I had to find time to fly back to Mexico and get a full diagnosis done. I am now back to being on my necessary medication that I was not able to obtain in Canada. I now finally have all the documentation I feel I would need to help prove my case. I have attached a scan from my doctors back home. Please let me know if require any further documentation
