Withdrawing and Returning to Studies

I have reviewed the information above and understand the terms and conditions.

I understand and agree to all the policies and regulations listed above.

I understand that submitting false information will be considered a departure from academic integrity and will be investigated accordingly.

I understand that I will NOT receive transfer credit(s) for courses taken at another post-secondary institution as part of a graduate program or while under a requirement to withdraw.

I understand that the Faculty Office may not be able to accommodate my program/plan request if my program is at capacity and/or I do not meet the eligibility requirements.

I understand that students who have been away for longer than 16 months are required to complete the Program and Plan requirements in effect at the time of their Return to Studies, and I agree to fulfill this obligation if this applies to me.
